Patient Participation Grouppatient survey

Would you like to have a say about the services provided at The Limehouse Practice?

The Limehouse Practice would like to hear your views.

The Friends of the Limehouse Practice Patient Group are encouraging patients to become a member of the patient group and to give their views about how the practice is doing.

We would like to be able to find out the opinions of as many patients as possible and are asking if people would like to provide their contact information so we can contact you every now and again to ask you a question or two about the services we are providing.

Are you interested in leaving your contact details?

If you could fill in this quick form below and either hand it back to reception, email it to

THCCG.friendsoflimehouse@nhs.net or provide your details over the phone, we will add your details on to a contact list.

Your contact details will only be used for this purpose and will be kept safely.

If you are happy for us to contact you occasionally by email please click the link below to open the sign-up form and complete all the fields.
